Description
Stavudine (formerly called d4T) is a synthetic thymidine nucleoside analogue that possesses virustatic activity against human immunodefi-ciency virus type 1(HIV-1) and type 2(HIV-2). Stavudine is phosphorylated intracellularly to stavudine triphosphate, which exerts antiviral activity. Because phosphorylation of stavudine depends on cellular rather than viral enzymes, conversion of the drug to the active triphosphate derivative occurs in both virus-infected and uninfected cells. Stavudine triphosphate inhibits HIV replication by two known mechanisms: 1. It inhibits HIV reverse transcriptase by competing with the natural substrate deoxy-thymidine triphosphate and 2. It inhibits viral DNA synthesis by causing DNA chain termination because stavudine lacks the 3'-hydroxyl group necessary for DNA elongation. In addition to the inhibitory effect on HIV reverse transcriptase, stavudine triphosphate inhibits cellular DNA polymerase beta and gamma, and markedly reduces the synthesis of mitochondrial DNA.
Indications
Stavir is indicated for the treatment of adults with advanced HIV infection who are intolerant of approved therapies with proven clinical benefit or who have experienced significant clinical or immunologic deterioration while receiving these therapies or for whom such therapies are contraindicated.
